Referrals
Make a referral to Safe Foundations QLD
We support children, young people and families across Queensland with assessments, youth mentoring, in-home family support, supervised contact and family group meetings. Use this page to understand what we need and how to refer.
How referrals work
A clear, respectful referral pathway
We aim to make referrals straightforward for families and professionals. If you’re unsure which service fits, send an enquiry and we’ll help you choose the right pathway.
1) Check eligibility & consent
Where possible, referrals are made with the family’s knowledge and consent. If there are court or statutory requirements, include relevant details so we can respond safely and appropriately.
2) Share key information
Provide the reason for referral, current risks and protective factors, key people involved, and any timeframes (e.g., court dates, reporting deadlines).
3) We confirm scope & next steps
We’ll confirm service suitability, expected timeframes, and what information we’ll provide back (e.g., progress updates, assessment reports).
4) Start support & review
We begin service delivery and review goals with the family and referrer (as appropriate), focusing on safety, wellbeing and practical outcomes.
